Community Transition Program

TRANSITION SERVICES

Training and Learning: Learn employability skills, specific job training skills, understand your disability and the impact on your learning or employment, and identify accommodations that can assist you.

Employment: Practice employability skills during school-sponsored work experiences and learn to write a resume, apply for a job, interview, and perform a job search.

Daily Living Skills: Learn and practice personal and home care routines, shopping, cooking, money management, and banking skills.

Community Access: Learn skills needed to safely access the community by bus. Identify and participate in community agencies and clubs that can help you in your job search or leisure activities.

Develop Resources: Learn about adult support agencies such as Developmental Disabilities Services and Vocational Rehabilitation Services and how to access these supports.

Recreation and Leisure: Explore new ways to use leisure time, join local clubs, and plan ahead so you can enjoy life.

  • We aim to create a transition experience where school supports are gradually replaced by other community supports over the course of 3 years. At the end of the program, the student exits school services with a strong team of community supports.

  • A key component of success is coordinating the supports of outside agencies. Staff work directly with students and families to apply for any outside agencies, including Vocational Rehabilitation, Chemeketa Occupational Skills Program, Summer Work Experience and Camp Lead.

  • Additional options are for student to gain guidance and support for planning on adult life. With instruction, students independently tackle the challenges of becoming an adult.
